Person Centred Plans for Older People

The continuing roll out of the Personalisation Agenda has seen the extension of Person Centred Plans for older people. Since the Valuing People strategy was introduced Person Centred Plans in learning disability services have been a key method for increasing people's self-determination and improving their own independence. Liverpool City Council employed AFTA Thought at an event this week to help launch a new initiative to demonstrate how the use of these plans has been making a difference to older people from across the city.

AFTA Thought are developing a series of workshops for delivery in May and June that will spread the message of Person Centred Planning to social care staff. The workshops will illustrate how these plans can help to support and reach out to those who's needs may be going unmet by more traditional service approaches.
